Donation Form

I am trying to find the donation form that the theme says it comes with. I looks like it is a Contact Form 7 form, I think, but I can not find where to edit it. Any information you can provide would be great!

Once I downloaded the dummy content a second time, then the form showed up. So it must not have completely downloaded all the content the first time. Maybe I reloaded the page in the middle of the process or something. Thanks!
